Case Management

During weekly one-on-one meetings with case managers, Village residents develop plans to help them obtain and maintain permanent income and stable housing. Services available through Case Management include intensive budgeting and monitoring of expenses, case plan accomplishment, referrals to internal and external social services, support for adult, vocational and parenting education, and follow-up services.
All residents in the Long-term Transitional Housing program are served by case managers.
Impact:
Each year, Case Managers help 350 homeless residents move on to independent, permanent housing, and 55% increase financial stability.
90% of families and 60% of individuals leaving the Long-term Transitional Housing program progress to successful independent housing – in total, 71% of Village Long-term residents moved to permanent housing.
